Consent for treatment
If you are attending the surgery for investigations, e.g. ultrasound, minor surgery, joint injection etc, then you will be asked to sign a consent form about the investigations and treatment.
These forms name the investigation or treatment you are having and records that you have had a discussion of your treatment with the practitioner.
You sign the form to acknowledge and consent that you have understood what the procedure is and what it is likely to involve.

The practice is hoping to be able to use telephone and mobile messaging more in the future. New patients and patients with certain illnesses are asked to consent to this type of contact.
