Consent for treatment

If you are attending the surgery for investigations, e.g. echocardiography, minor surgery, joint injection etc, then you will be asked to sign a consent form about the investigations and treatment.

These forms name the investigation or treatment you are having and records that you have had a discussion of your treatment with the practitioner.

You sign the form to acknowledge and consent that you have understood what the procedure is and what it is likely to involve.
